The information below is required prior to all appointments with new clients and must be verified prior to our meeting. The fields which asks for a provider reference are for a provider you've seen in the past with name,website and emails address.
One thing I need to remind you of is that I have 2 wonderful children and a life so when contacting me I will confirm our appointment personally via E-Mail or phone call so you will know that our time is scheduled. There are times that you may email me and I am out of town or not able to check my mail so please don't assume that our time is set until you hear back from me, not an email stating your mail has been sent to me from here. One last thing, remember it is difficult for me to schedule same day appointments but when possible I will. Let's get together SOON!
